Use this form to request an appointment with us. While we will do our best to accommodate your requested day and time, please note, your appointment is not fully booked until you get a confirmation from us as we must check our availability!

Please note that we are generally booking new client appointments 1-2 weeks out.  Urgent-new cases may be accommodated based on our availability.  Holidays and Covid-19 may greatly impact appointment availability as well.
